HS 219 Client Records, Case Management, and Care Coordination Lecture Hours: 3 Credits: 3
Covers the preparation of clinical documentation related to screening and intake processes, assessments, treatment plans, reports, progress notes, discharge summaries, using medical/behavioral health terminology and other client-related data and care coordination. Applies State, HIPAA, ASAM, and other professionally relevant standards.
Prerequisite: HS 150 and HS 170 , each with a grade of C or better; or consent of instructor. Recommended: Concurrent enrollment in HS 284 - HS 288.
Student Learning Outcomes:
- Explain accepted principles of client record management including the importance of protecting client confidentiality.
- Identify and track the essential components of client records, including release of information forms, assessments, service (treatment) plans, individual service notes, transition and discharge summaries.
- Identify current Federal, State, local, and program regulations related to client documents.
- Employ the basic rules of correct grammar in preparing clinical documentation.
- Compose clear, legible, and concise records that comply with agency standards and relevant administrative rules.
- Synthesize and summarize client information in writing using electronic medical records system, and utilizing clinical terminology.
- Recognize and apply medical and behavioral health approaches.
- Identify and discuss case management and care coordination processes, and service support activities.
- Correctly complete release of information forms.
- Apply infectious disease regulations with respect to client confidentiality.
