BH 266 Case Management and Care Coordination

Provides theory and application in care coordination, systems navigation, casework and interviewing applied to diverse populations and cultures in behavioral health settings. Includes interviewing for assessment, problem solving, planning, monitoring, crisis intervention and development of a case file.

Prerequisite: BH 203   and BH 219   both with a grade of C or better; or consent of instructor. 
Concurrent: BH 216  
Student Learning Outcomes:
  1. Describe case management, care coordination, and systems navigation skills applied in behavioral and community health settings.
  2. Understand interviewing and assessment skills in all phases of case management, care coordination and systems navigation.
  3. Apply principles of service coordination to case management, care coordination, and systems navigation.
  4. Apply documentation standards to  casework management, care coordination, and systems navigation.
  5. Identify and discuss tool for time management and organization as they relate to record management, case management, and care coordination processes.
